Go365 is constantly updating our program to meet the needs of our members! We are excited to introduce a new submission process when you complete biometric-related activities. Starting 8/18/2018, this includes:


  • Biometric screening
  • Blood glucose, blood pressure, cholesterol and weight
  • Nicotine test


Acceptable proof would include:



**Please note: Your full name and date of service are required to be listed on the copy of your proof. If this information is not present it will not be accepted. All submissions are subjected to random audits from our team.


Starting now, to earn Points for these activities follow these steps:


1. Select Activities from your Go365 Dashboard

2. Select Prevention

3. Find the Activity you would like to submit for ex. Biometric Screening


4. Select View Details

5 Scroll down to step 3, select the type of screening you would like to submit

6. Enter in your information in the spaces provided

7. Enter the date of your screening


8. Select choose file to upload your documentation

9. Submit


10. If your information was received successfully, the message below will display as a confirmation. Please note: if you do not receive this confirmation or receive an error, please retry refreshing your screen or re-entering your information

Important Note: All activities, screenings and submissions, are subject to an audit process as defined by the Go365 Terms and Conditions. Submissions are checked for accurate information and determined if they meet the requirements for each individual activity or screening. 


If the proof is determined ineligible they will contact you through phone, email or letter to provide education. Often, it can be a missing page, name and date or the image came through blurry and they are unable to verify the information. If you initially submitted invalid proof and the activity was denied/reversed, but have valid proof, you can resubmit with the valid proof.
