Time is running out!

If you chose a LivingWell health plan for 2016, you must complete the Go365 Health Assessment or get a biometric screening by July 1.


Health assessment

Visit and sign in to Go365 to get started. You do not have to get a biometric screening before taking the health assessment.

Biometric screening

  • Screenings are offered at no cost to you at a number of convenient locations. Find locations here.
  • your (HumanaVitality/Go365) ID card, or obtain your Go365 ID online or on the Go365 App.
  • Fast for at least eight hours prior to your appointment.
  • Keep your screening results for immediate proof of completion.



Earn up to 4,000 Go365 Points. Receive even more for completing both the online health assessment and biometric screening.

What if I don't?

If you don't complete the LivingWell Promise (either a health assessment or biometric screening), you will not receive the discounted insurance premium ($40) for a LivingWell plan in 2018.


Go365 customer service hours are Monday-Friday 8 a.m. - 7 p.m. EST. July 1 is a Saturday, and there will be no customer service representatives available if you should need assistance that date. A maintenance outage of Go365 is also planned from approximately 10:30 p.m. EST June 30 until 2:30 a.m. July 1.

What:  KEHP members may not be able to access their HumanaVitality tab when logging into our website.  Members may see “Unavailable” errors when clicking through various links.

Who:  This affects Kentucky Employee Health Plan members enrolled in HumanaVitality.

When:  These issues started the morning of Wednesday, January 13.

What Next?  Our IT team is aware of the matter and is working on it.  They expect to have access restored within 48 hours. 

What should I do?  You don’t need to do anything at this time.  There is no need to contact the HumanaVitality customer care team as this is a known situation.  We appreciate your patience while we work this out for you.  Please feel free to spread the word to your colleagues if they are also are experiencing this issue.
