I recently had a name and address change and am not able to get into my go360 account.  Please advise how I can access my account again since I cannot remember the information my original account was setup. slambert3@humana.com

I can log into Humana.com and it shows no activity in my go360 account.  I've let this problem go on way too long now.  I am not able to log into my go36o account at all.  When I select forgot my username to enter my information, it does not recognize any information I enter. Should I try setting up a new go360 account or it there some tech support that can help me sign into my existing account?  slambert3@humana.com
