I mailed my vision exam results and the doctor’s signature quite some time ago. I do not see where it has been creditors to my account.

If you have a copy of your proof, you can easily resubmit your vision exam via your Go365.com account, or mobile app.

Hello. Thank you for your reply. Yes, I did include my member ID number. I used a Go365 Prevention Activity Form, filled it out completely, had the doctor sign it and give the results to me. I mailed it in and forgot about it. I thought I was done with it. When I did think to ask you, I sent this email. I used a Live Chat just now and they tell me I have forfeited any available points because I submitted it just today (they told me how to do it online, too) and it’s now out of date. That stinks.
