Every time I try to log in I have to get a confirmation code.

I've had the same problem since two-factor authentication was implemented... it continues to happen on both my laptop and cell phone. I've made sure that I'm not clearing cookies or cache on exit, etc. It is VERY frustrating!
I have had the same issue since November 2019. Every time I log in on the android app. It says it's a new device and that I need to plug in the confirmation code it emails to me. I reinstalled the app a few times but same story. I put in an IT ticket and they reset my account so I was able to at least change my email address to my phone but it still required that I input an confirmation code. I used to use the app all the time, not so much anymore. Samsung Galaxy S7 Edge. It was working great until November update.
