It shows my health assessment as 0%. I cannot seem to figure out how to work or navigate this app and my assessment is due in 3 days. I am using a Samsung 9 plus.

After reviewing your account, I can see that it shows your Health Assessment has been completed as of the middle of last week and your Points are awarded to your account.
If you are still not showing it completed, can you please provide a screenshot from the Go365 Mobile app for further review? I would be more than happy to take a closer look!
