Unable to Log In - Phone and Website

I am unable to log in via my app or on the website.  It asks to verify my device every time with a code and when I do, an error message pops ups and I am unable to enter the Go365 portal.  This has happened all week.  My device has also never saved to the app, so I have to get the code every time that I log on.
